How to answer FAFSA® preparer information questions

The FAFSA® preparer section is only for someone other than the student or parent/guardian who completed the form. Preparers must enter their identity, SSN and EIN, organization and mailing address, then sign and date the form to confirm the answers are accurate.

The preparer needs to complete and sign the section to verify they’ve answered all the questions as honestly and accurately as possible.

Can I skip these questions?

  • You can skip if you’re the student or the parent/guardian. This section is only for preparers to fill out.
  • Preparers may not skip any of these questions.

How to answer these questions

If you prepared the FAFSA® for someone, you’ll need to:

  • Enter your name exactly as it appears on your Social Security card.
  • Enter your Social Security number and Employer Identification Number (EIN).
  • Enter the name of your affiliation or organization.
  • Enter your permanent mailing address.
  • Sign and enter the date you signed.
